Working in partnership with Channel Health Alliance, Invicta Health provides a Home Visiting Service across the South Kent Coast CCG area. Following direct communication and triage between clinicians at the practice and within the Home Visiting Team, practices are able to access the service which provides visits to patients at home. Following a visit by a member of the team, a full handover and communication is reported back to the practice so that continuity of care remains throughout. The team consists of Paramedic Practitioners, Nurses and Health Care Assistants working within their individual scope of practices and is operational on weekdays from 08:00-19:00.
The visiting service is an extension of the hub service, offering urgent (on the day) visits to house bound patients requiring clinical assessment and treatment. The visiting service will be provided by an multi-disciplinary team consisting of Paramedic Practitioners, senior Nurses and HCAs. Visit records will be maintained using GP practice systems linked via third party software.
The service will be led by a senior Paramedic Practitioner and lead Nurse will provide clinical leadership to nursing staff. The service will report to the Chief Executive and clinical oversight will be given from Director of Nursing and Clincal Director.
The service will work closely with the locality leads within South Kent Coast CCG to ensure the service meets local priorties. Support services will be provided by our central services based in Canterbury.
